Overview We’re looking for a Hospice Social Worker (MSW) – Volunteer Coordinator to join our compassionate team. You’ll play a dual role: supporting the emotional and psychosocial needs of patients and families, while leading the volunteer program to ensure timely, personalized service. This role reports directly to the Executive Director or Administrator and offers the opportunity to make a difference both at the bedside and behind the scenes.
As a Hospice Social Worker (MSW) – Volunteer Coordinator, You Will:
Perform psychosocial assessments, case management, education, and follow‑up for hospice patients and families
Provide counseling, support, and crisis intervention to individuals and families navigating end‑of‑life issues
Address long‑term care planning, advanced directives, and cultural/spiritual dynamics
Monitor and ensure a safe home environment for patients
Advocate for volunteer services and oversee the coordination and supervision of all volunteer activities
Assess requests for volunteer assistance and assign volunteers accordingly
Conduct volunteer training, maintain personnel files, and monitor visit documentation and compliance
Serve as the liaison between volunteers and the clinical care team
About You Qualifications – What You’ll Bring:
Master’s degree in Social Work from a CSWE‑accredited institution
Licensure as required by the state in which the hospice is located
At least one year of experience in a healthcare and/or hospice setting (three to five years preferred)
Deep understanding of the psychosocial dynamics of illness, loss, and death
Compassionate and tactful when supporting patients and families facing terminal illness
Ability to provide education, resources, and emotional support during times of high stress
Comfortable supervising others and coordinating multi‑person teams (volunteer or clinical)
Organized and able to manage documentation, compliance, and volunteer engagement processes
Preferred Experience (Not Required)
Hospice‑specific experience as a Medical Social Worker
Experience leading or developing volunteer programs
Familiarity with EMR documentation systems and CMS regulations
Previous work supporting interdisciplinary teams or case management
Cultural competency in supporting diverse populations at end‑of‑life
