Receive 17% Weekday Nights, 26% Weekend Nights and 15% Weekend Day shift differentials.

Minimum Offer: $17.86/hr. Maximum Offer: $26.78/hr.

Department: CPC Youth Intensive OP‑Pro Fee. FTE: 1.00. Full Time. Shift: Evenings. This position supports the Youth IOP operating Monday‑Friday 11:00am‑7:30pm.

Position Summary Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency. Identify and address strengths and barriers to facilitate independent functioning in the community. Support crisis situations and provide personalized interventions. The patient care assignment may include children at risk of serious emotional, neurobiological, and/or behavioral disorders; adults with severe mental illness; individuals with substance abuse or co‑occurring disorders. Ensure adherence to hospital policies. Patient care may include neonate, pediatric, adolescent, adult, and geriatric age groups.

Detailed Responsibilities

Follow patient safety‑related policies, procedures and protocols.

Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety.

Identify and report/correct environmental conditions that may put a patient at risk.

Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner.

Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don’t understand or may not seem right.

Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice.

Work to eliminate barriers to access to culturally competent medical services for all patients. Assist patients to access community resources and financial assistance programs.

Collaborate with medical providers, multidisciplinary team members and community resource providers to assist patients in meeting treatment goals.

Facilitate and encourage the development of skills in activities of daily living, interpersonal coping, socialization, and community functioning.

Plan for professional growth related to professional goals based on self‑assessment, evaluation and feedback.

Offer information and resources about the patient’s specific mental condition; serve as a mentor to promote recovery and resiliency; teach symptom monitoring, management and relapse prevention skills.

Assist individuals with medication knowledge, side effects, and medication concerns; facilitate self‑motivational skills for medication regimen.

Assist in the development and coordination of the individual’s service plan.

Assist the client in obtaining access to necessary rehabilitative, medical and other services.

Encourage the development and succession of natural supports.

Work with the individual/family to identify personal strengths, needs, and barriers to attaining self‑identified goals; conduct ongoing assessments.

Assess, support and intervene in crisis situations; facilitate crisis management plans.

Coordinate and monitor use of services, track client activities, maintain contact with providers, conduct post‑discharge follow‑up.

Document all client encounters; complete and submit billing documentation; maintain comprehensive client files.

Maintain and report applicable statistics regarding programs and client services.

Enhance professional growth through educational programs, reading literature, attending in‑services, meetings and workshops.

Qualifications Education (essential):

High School or GED Equivalent.

Experience (essential):

1 year directly related experience.

Credentials (essential):

CPR for Healthcare/BLS within 30 days; Certified Peer Specialist recognized in State of NM; Valid New Mexico Driver’s License; UNM Vehicle Operator’s Permit within 60 days.

Non‑essential:

Bilingual in English and Keres, Tewa, Tiwa, Towa, Zuni, or Navajo.

Physical Conditions Light Work: Exerting up to 20 pounds of force occasionally, or up to 10 pounds frequently; may require walking, standing, pushing or pulling.

Essential working conditions:

Medical Hazards: Dirt, dust, fumes, odors, bad weather, noise.

Subject to random alcohol and substance testing.

Tuberculosis testing completed upon hire and as required.
