$42.00 - $46.00 per hour (Based on Experience)

Position Overview: The Medical Social Worker (LCSW-C) is responsible for implementing the plan of care for specific patients, beginning at admission and continuing through discharge.

Location: Howard County Medical Center located in Columbia, MD

Schedule: Monday- Friday 10:00 AM - 6:30 PM

Responsibilities:

Provide age‑appropriate, patient‑centered care across all age groups. (Birth - 70 years and up)

Complete timely psychosocial assessments and manage high‑risk cases (e.g., re‑admissions, elderly, uninsured, homelessness, abuse/neglect, psychiatric and substance use concerns).

Collaborate with the healthcare team to coordinate continuity of care and discharge planning.

Connect patients and families with community resources and advocate across inpatient, outpatient, home, and community settings.

Facilitate APS/CPS reports, guardianship, advance directives, and hospice referrals.

Provide crisis intervention and support patient/family decision‑making.

Educate staff on psychosocial issues and participate in interdisciplinary rounds and departmental meetings.

May supervise LMSW social workers or social work students.

Qualifications: MS Social Work from an accredited school of social work required.

Must be licensed by the Maryland Board of Social Work Examiners at the LCSW-C level.

1 year of experience in providing clinical social work service, preferably, in a acute medical care setting.
