The Nursing Home Placement Coordinator provides a centralized resource for all Nursing Home referral and discharge activities. The role is essential for communication and planning for post‑discharge care in a Nursing Home setting. Excellent communication with unit interdisciplinary teams and Nursing Home Admissions teams is critical to our hospital throughput.
Essential Functions
Provide direct and indirect services to patients who need placement in a post‑hospitalization after‑care facility.
Coordinate communication with nursing homes regarding patient care needs, financial profile and other components of the patient’s application or admission.
Provide placement consultation to Social Work staff, community facilities, community or hospital physicians, other health care professionals, and, when indicated, to patients and/or families.
Maintain Social Work Division liaison relationships with area nursing facilities and insurers.
Receive and review initial or updated information from SMH Social Workers intended for placement purposes, and transmit the information to appropriate community facilities.
Negotiate with individual facilities; collaborate with Hospital interdisciplinary team, patient and family, as needed, to ensure optimum, timely placement of all patients.
Problem‑solve on case‑specific or system issues with Social Work Senior Director, leaders and individual Social Workers, or others as indicated.
Facilitate the Placement Program’s inpatient throughput.
Ensure the Program’s compliance with regulatory requirements & provide education to interdisciplinary team, patients & families.
Collaborate with leadership to enhance the Program’s responsiveness to the Hospital’s bed availability needs.
Maintain a high level of “customer satisfaction” with the Program among patients/families, staff, and nursing homes.
Serve as Placement Specialist for all patient care units within the Hospital.
Consultation and planning with unit Social Work staff to determine the optimal placement strategy.
Initial case review and analysis of new placement referrals to prepare for transmittal.
Transmittal of written referral information to nursing homes by ePartner/email/courier/fax.
Provision of information to Hospital Social Work staff regarding nursing home evaluation results and issues of concern.
Documentation of patient information for entry into eRecord.
Coordination of daily nursing home bed availability.
Negotiation with nursing homes to secure beds.
Participation in daily census review meetings and activities.
Consultation regarding facility selection or placement issues with unit Social Workers, patients and/or families to be discharged home to await placement.
Consultation and education with unit social workers regarding complaints received from the nursing facility after admission.
Coordinate OPWDD and mental health Level II determinations required by NYS (except in Behavioral Health).
NY State screen trained and complete screens for pediatric population requiring nursing home placement.
Administrative
Oversight of Program operations to determine the level of compliance with Division placement policies.
Ensure effective Program operations.
Analyze regulatory updates to project the impact on patient movement and determine the need for revision of Program policy or procedures.
Collaborate with Manager to address areas of Program operations not in compliance and to develop necessary revisions in Program policy or procedures.
Maintenance and analysis of statistical data relating to Patient Placement.
Generation and distribution of special reports as requested by leaders.
Collaborate with Manager in developing proposals to obtain Hospital‑based supports needed by the Placement Program.
Assist the Manager, as requested, in developing collaboration initiatives with other hospitals in the community regarding placement issues.
Maintenance of updated information files on all area nursing facilities and appropriate out‑of‑area facilities.
Document patient complaints regarding NH’s and address them with NH’s and SMH leadership.
Participate in activities of the Social Work Division including, but not limited to, timely completion of statistical reports/forms, time sheets, daily on‑call, Hospital and Division mandatory in‑service training, University Health Services reviews.
Participate in relevant training and/or educational opportunities designed to enhance knowledge and skills.
Knowledge of and compliance with all policies of the Social Work Division and Strong Memorial Hospital.
Teaching
Orientation of new Social Work staff to the Placement Program and workflows.
Development of placement resource materials to assist Social Work staff.
Preparation and presentation of periodic in‑service training sessions on placement issues and SNF’s for Social Work staff.
Minimum Education & Experience
BS or BSW with no experience OR no Bachelor’s degree but 4 years of experience.
Experience in a health care setting preferred.
Knowledge, Skills and Abilities
Excellent communication skills (written and verbal); patient‑focused, advocacy skills and interdisciplinary collaboration.
