Position Title:
Licensed Master Social Worker (LMSW) / Licensed Clinical Social Worker (LCSW) – Hybrid Opportunities
Location:
Bronx, NY (Hybrid: 1 day onsite, 4 days remote)

Job Summary
The Licensed Master Social Worker (LMSW) / Licensed Clinical Social Worker (LCSW) will be responsible for delivering high-quality social work services in a hybrid care model. This role involves conducting patient assessments, providing evidence-based behavioral health treatments, and collaborating with interdisciplinary care teams to ensure comprehensive patient care.
Responsibilities
Conduct assessments of patients referred by Primary Care Providers (PCPs) using evidence-based tools.
Deliver treatment for behavioral health conditions using approaches such as
CBT, Motivational Interviewing, crisis intervention, Interpersonal Psychotherapy, and behavioral activation .
Provide treatment recommendations during collaborative care team meetings.
Monitor patient progress through care planning and coordination with clinical teams and health homes.
Deliver crisis intervention services and risk assessments as needed.
Engage clients with limited behavioral health service utilization through outreach efforts.
Document all services in a timely and accurate manner using electronic health records.
Participate in weekly clinical supervision, team meetings, and case conferences.
Qualifications
Active NY LMSW or LCSW license
Master of Social Work degree
from an accredited institution
Minimum 2 years of experience
in social work or behavioral health
Spanish required
Compensation
LMSW:
$70K – $75K
LCSW:
$90K – $95K
