Baptist Health is seeking a compassionate and motivated Social Worker II to join the Behavioral Health team at Wolfson Children’s Hospital. This PRN Days position plays a vital role in supporting pediatric patients and their families by addressing psychosocial needs and ensuring access to essential services and resources.
Key Responsibilities
Assess patients’ psychosocial needs to support emotional well‑being and optimal functioning.
Provide discharge planning, including coordination of home health services and durable medical equipment (DME).
Assist with patient placement and transfers, ensuring smooth transitions of care.
Offer referrals to community resources to support ongoing care and family needs.
Support utilization review activities to promote effective and appropriate care delivery.
Education
Master’s Degree (required)
Doctorate (preferred)
Experience
1-2 years Counseling Experience - required
1-2 years Experience in Child Welfare Services - required
1-2 years Social Work Experience - required
1-2 years Hospital Experience - required
Less than 1 year Experience in Behavioral Health Setting - required
License & Certification
Licensed Clinical Social Worker (preferred)
Driver’s License (preferred)
