Bishop McCarthy Center For Rehabilitation & Healthcare is looking for a Social Worker (LSW/CSW) to join our team. We are a team‑oriented facility committed to making our residents and staff comfortable and happy. Your role will focus on supporting residents through emotional and social needs, ensuring high quality care and smooth transitions.

Responsibilities
Assists residents with emotional adjustment to facility.
As directed, interview new residents and record social history and assessments.
Completes the social services portion of the MDS, care planning, baseline, etc.
Refers to psychological services as needed.
Schedules and attends interdisciplinary care plan meetings (morning meetings, utilization review, etc.).
Writes summaries and progress notes as directed.
Assists in conducting a pre‑discharge evaluation including setting up Visiting Nursing Services and community resources.
Assists with discharge planning and follows up after discharge.
Conducts required Cut Letters.
Writes discharge summaries as assigned.
Develops a professional relationship with each resident and family.
Educates residents/representatives on Advanced Directives.
Adheres to all Social Services Regulations.
Qualifications
Minimum Bachelor's Degree in Social Work from an accredited college.
Must obtain a CSW and/or LSW.
One to three years of professional experience and/or training; or equivalent combination of education and experience.
