We are seeking a detail-oriented Hospice LPN Scheduler to coordinate hospice patient care schedules and support the intake and referral process. This role ensures timely, accurate scheduling of clinical staff and oversight of referral documentation to support smooth admissions and continuity of care. The ideal candidate is an LPN with strong organizational skills, excellent communication abilities, and a commitment to patient-centered, compliant hospice operations.

Primary Responsibilities

Coordinate and manage schedules for hospice nursing and clinical staff to ensure timely patient visits.

Assist in the intake and referral process, ensuring all required clinical documentation is complete, accurate, and compliant prior to admission.

Review referrals for appropriateness and readiness for scheduling; communicate with referral sources for missing or clarifying documentation.

Maintain accurate records of patient schedules and staff assignments.

Communicate effectively with patients, families, and the interdisciplinary team regarding visit times, changes, and updates.

Assist with transitions of care as needed.

Identify and address scheduling conflicts or urgent needs to support uninterrupted patient care.

Collaborate with clinical and administrative teams to optimize workflow, ensure regulatory compliance, and support quality outcomes.

Maintain confidentiality and compliance with organizational policies, state regulations, and Medicare hospice guidelines.

Qualifications

Current, active Washington State LPN license in good standing.

Minimum 2–4 years of clinical experience in healthcare.

Experience in hospice, home health, intake/referral coordination, or healthcare scheduling preferred.

Strong attention to detail and accuracy in reviewing clinical documents.

Excellent verbal and written communication skills.

Proficiency with EMR systems, scheduling software, and Microsoft Office Suite.

Ability to multitask, prioritize, and work both independently and collaboratively.

Work Schedule

In-office: Monday – Friday, 8:00 a.m. – 5:00 p.m.

Additional coverage: Weekends as needed

Compensation $32.00 - $35.00/hour
