Health Facilities Surveyor – State of South Dakota, Department of Health - Division of Licensure and Accreditation. Location: Pierre or Rapid City, SD. Salary: $26.61-$31.31 hourly, depending on qualifications. Pay Grade: I. Closing Date: 1/20/26. This is a full-time position. This position involves travel, a daily per diem, and the use of a state vehicle while conducting surveys. Working from home is possible up to 3 days a week (while not on survey) after a 6-month probationary period, subject to position requirements. The Health Facilities Surveyor is responsible for surveying patient activities, dietary, infection control, resident rights, and the physical environment to ensure compliance with state and federal regulations for the protection of health and safety hazards. Survey work includes participating in pre-survey planning, conducting inspections, and reporting findings. The role requires travel, including overnight stays, and may involve working in a state vehicle; per diem is provided. Licenses and Certifications

The candidate must be licensed related to their professional role, if applicable. Training for the survey process includes online Centers for Medicare and Medicaid Services (CMS) courses and precepted surveys in licensed healthcare facilities. The Ideal Candidate Will Have
