Responsibilities
Responsible for Nursing Home Case Management
Conducts psychosocial assessments, prepares & updates social histories on residents & completes all paperwork required relative to services provided
Assists supervisor in evaluating current programs & treatment plans
Visits nursing units to talk to residents regarding their complaints
Develops intervention goals & objectives designed to give education & resolve resident’s problems in functioning
Monitors resident’s response to treatment plans & provides feedback on effectiveness of prescribed treatment
Assists Interdisciplinary Team (IDT) in planning & providing special programs for residents
Prepares, reviews, & updates social histories & assessments for required documentation for nursing home licensure, Veterans Administration regulations & Ohio Dept. of Health requirements
Attends workshops, conferences & training sessions to remain current on social service cases, problems & techniques
All other duties as assigned
Qualifications & Requirements
Requires current license as social worker (i.e., LSW) as issued by State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board in accordance with ORC 4757.28. Pursuant to ORC 4757.41 (A)(5) any person engaging in social work or professional counseling as a civil service employee as defined in section 124.01 of the Revised Code who has at least two years of service as of July 10, 2014 is exempt from current licensure as social worker (i.e., LSW) requirement.
