Job Summary

Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.

Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.

Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.

Qualifications

Education: Master's Degree in Social Work (required) or Master's Degree in Mental Health & Behavioral Medicine (required).

Licenses and Credentials: Licensed Clinical Social Worker (LCSW) [State License], Licensed Independent Clinical Social Worker (LICSW), Licensed Social Worker (LSW), Licensed Mental Health Counselor (LMHC) – each preferred.

Experience: Clinical experience in a medical setting (0-1 year required).

Skills and Abilities: Excellent organizational and time‑management skills; crisis intervention skills; problem‑solving and conflict resolution; ability to work collaboratively and independently; strong written and verbal communication; knowledge of community resources and aging process; fundamental skills in risk assessment and psychosocial treatment plan development.
