Position Summary: To screen, assess, and evaluate patients and their families to identify the social and emotional impact of patients living with a terminal illness. To develop interventions to increase coping with an impending loss and continuously assess for comfort and safety as the patients and caregivers’ needs change.
Education Master's degree in social work from an accredited program required, New Jersey Social Work License required.
Experience Previous home hospice social work experience preferred.
Skills
New Jersey Social Work License required. Effective oral and written communication skills to express ideas clearly.
Effective listening skills. Ability to follow through using appropriate channels of communication. Ability to pay attention to details, and remain efficient when switching tasks.
Function well in stressful situations and changing work environment related to patient needs, including working with patients with acute, chronic, and complex disease processes.
Work cooperatively with patients, family members, and multidisciplinary team members (i.e. nurses, physicians, rehab, chaplains, HHAs, art therapist, etc.).
Utilize effective time management to set priorities, perform job-related responsibilities, and respond quickly to emergency situations.
Use effective analytical and critical thinking skills to problem solve.
Driver's License: current and valid driver's license, registration, inspection sticker and appropriate insurance coverage required.
