Job Details Salary Range:
$40.00 - $42.00 per hour
Schedule:
Thursday through Monday with alternating weekends
General Responsibilities Plan, organize and manage clinical operational aspects of the Reflections program under the oversight of the RN Wellness Director, ensuring the highest level of dementia care through implementation of INVEST mission and Brandywine standards.
Ensure compliance with all federal, state guidelines and Brandywine policies and procedures.

Is knowledgeable of resident rights and ensures privacy, dignity, and well-being in a safe, secure environment.
Supports, cooperates with, and implements procedures and programs for safety, universal precautions, fire/safety/disaster plans, risk management, and security.
Maintains confidentiality of all data, including resident, employee and operations data.
Ensures Quality Assurance and compliance with all regulatory requirements.
Complies with law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior.
Supports common teamwork: cooperates with all co-workers; plans and completes job duties with minimal supervision.
Uses tactful, appropriate communications in sensitive and emotional situations.
Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems and concerns.
Promotes positive public relations with residents, family members and guests.
Completes requirements for in-service training, attendance, uniform and dress codes, personal hygiene and other duties as assigned.
Essential Duties
Coordinate recruitment and hiring process for Reflections Care Management team.
Complete staff performance appraisals/disciplinary actions.
Facilitate orientation for all staff on the Reflections Program.
Conduct monthly in‑services on various dementia‑related topics for all staff.
Perform environmental rounds to ensure safety of exit doors, resident rooms and unit adherence to brand standards.
Provide direct support to care‑management staff for behavior symptom management and resident care approaches.
Coordinate and facilitate the RCP Training program for primary staff on unit; track completion and schedule RCP classes.
Suggest residents for restorative potential and ensure program consistency.
Cooperate with other resident services for complete evaluation of care.
Assist in establishing linkages and arranging services from public and private sources for income, health, mental health and social services.
Assist residents in applying for and maintaining income entitlements and public benefits.
Coordinate care planning per state regulations.
Implement and monitor resident service plans with the Wellness Director.
Utilize Brandywine standards and professional organization standards to conduct resident care.
Demonstrate knowledge and skills to safely provide care appropriate to residents' age.
Cooperate with other resident services when coordinating nursing services.
Assist with admits, transfers, and discharges of residents as required.
Report to the RN to assess and document resident and family response to interventions.
Provide written and/or oral status reports of residents to ensure needful follow‑up.
Requisition and arrange for diagnostic and therapeutic services per physician orders.
Assist the physician on rounds and ensure effective communication.
Assist in resident rehabilitation activities, ambulation assistance, and emotional support; monitor physical therapy results.
Provide direct physical and psychological care in emergencies or illness.
Report all observations and changes in condition to the RN.
Prepare and administer medications per physician order and medication administration standards.
Ensure adequate supplies and equipment are available for nursing needs.
Comply with Controlled Substances policy and maintain accurate inventories and controlled logs.
Review medication administration records for completeness, accuracy, and adherence to stop‑order policies; report discrepancies.
Dispose of drugs and controlled substances per established procedures.
Monitor residents, report to RN, and take appropriate action as instructed for side effects or adverse reactions.
Adhere to infection control policy and procedures.
Administer and document all treatments per policy and procedures.
Maintain accurate resident care records and document pertinent data regarding the nursing process.
Assist in developing resident care assignment sheets.
Review all resident care manager assignments for completion.
Work with INVEST Coordinator to ensure a therapeutic and socially engaging environment.
Qualifications Education/Training:
Licensed Practical Nurse with successful graduation from an accredited nursing program and unencumbered LPN license. Training in dementia care desired.
Experience:
Previous experience with a dementia population preferred. Experience in hospital, assisted living, skilled nursing or related medical community required.
Working Conditions:
Moves intermittently during working hours with frequent interruptions. May encounter hostile and emotionally upset residents, family members, personnel and visitors. Must cope with mental and emotional stress of the position.
Physical and Sensory Requirements:
Knowledge of federal and state regulations, nursing practices, techniques and methods applied to health and wellness residents.
Proficient in reading, writing, grammar, mathematics; strong interpersonal and communicative skills; good auditory and visual skills.
Will be required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
Must lift or carry up to 50 lbs intermittently and use proper body mechanics.
