Health Care Coordinator RN PRN Under the supervision of the Clinic Supervisor, the Health Care Coordinator is responsible for completing routine treatments including medication passes and infection control. Clinic Duties: The Health Care Coordinator utilizes the nursing process including; assessment, planning, implementation, evaluation, and recording of participant care and participant education. Provides support to care management team by anticipating the needs and providing assistance in the provision of care and implementation of medical treatments. Coordinates services with other members of the health care team, internal and external, to provide complex participant care. Answers or returns phone calls, providing on-site and telephone triage as needed. Provides direct and indirect nursing care to assigned participants. Works under the supervision of the Clinic Supervisor or designee, but is independent in the application of advanced nursing knowledge and skills. Practices within the scope of the North Carolina Nurse Practice Act and PACE policies, procedures, and standards. Maintains inventory of supplies and medications; orders as needed. Ensures quality assurance measures are achieved. Serves as the primary infection control nurse for the center. Performs surveillance of infection related data. Participates in quality improvement processes and analyzes data related to infection. Develops and implements interventions to reduce infections in the center. Minimal Education: Graduate of accredited school of nursing.Bachelor’s degree of Science in Nursing is preferred. Current NC RN License Experience Minimum of three years work experience as a registered nurse required. Minimum of one year experience working with the frail and elderly population.One (1) year home care, long term care, or PACE experience preferred. Experience in an acute care setting (ICU or ER) is
preferred.Experience
