Registered Nurse (RN) – IDD Host Home Program Location:

Austin, TX (Hybrid – Remote After Training)

Salary:

$75,000–$80,000 annually, based on experience

Schedule:

Monday – Friday, 8:00 AM – 5:00 PM

Position Overview This Registered Nurse position supports our Host Home Program, which is structured differently from traditional nursing roles. This is a case management–focused RN position with an emphasis on coordination, monitoring, and oversight of services rather than daily hands-on clinical care.

After an initial training and onboarding period, this role is primarily remote, with the expectation of coming into the Austin office one day per week to complete tasks that must be handled onsite. Office expectations may be adjusted based on performance and independence in the role.

The RN manages a caseload of approximately 50 individuals, working closely with the Program Director and traveling regularly to ensure quality care and compliance.

Key Responsibilities

Conduct comprehensive nursing assessments at least

quarterly , and as needed based on changes in condition

Implement, monitor, and update individualized health care plans

Coordinate and monitor medical and support services for assigned caseload

Communicate with physicians, specialists, pharmacies, and external medical providers to ensure continuity of care

Provide medical oversight, technical assistance, and training to Host Home providers and staff

Ensure physician orders are properly documented and carried out as prescribed

Provide input into individual service plans related to health care needs

Travel with the Program Director on a regular basis to visit all individuals at least quarterly

Maintain accurate documentation in accordance with program and regulatory requirements

Work Environment & Travel Expectations

Caseload is primarily located in North Austin, Temple, and Burnet

Ideal candidate will reside in the North Austin area for easier access to assigned individuals

RN will use their personal vehicle for travel (often riding together with the Program Director)

Mileage reimbursement provided for all work-related travel

During the initial employment period, the RN will spend significantly more time in the Austin office for training, oversight, and support

As independence increases, the role transitions to mostly remote work

What You Will Bring

Graduate of an accredited Registered Nursing program

Current Texas RN license in good standing

Minimum of one year of experience working with the IDD population

Strong assessment and critical-thinking skills

Experience coordinating and monitoring services preferred

Excellent communication skills with the ability to collaborate with medical professionals, team members, individuals served, and families

Current driver’s license, valid car registration, and auto insurance

Current CPR and First Aid certification
