An exciting opportunity to join the Corporate Complex Discharge Team as a Discharge Co‑ordinator. The team supports patients identified as needing support from partner organisations for safe, timely discharge.

Key responsibilities include:

Collaborate with multidisciplinary teams to facilitate effective, efficient and safe discharges.

Initiate discharge planning from admission, using SBAR assessments.

Champion the #HomeFirst philosophy and support clinical & non‑clinical workforce.

Understand internal escalation processes and support proactive organisational response to demand.

Essential responsibilities:

As part of the Corporate Discharge Team and Ward Clinical Teams, you will support patients with discharge plans, avoid hospital admission, and drive discharge care planning from admission onward.

Required skills and experience:

GCSE level education in a healthcare setting; NVQ4 or foundation degree or equivalent.

Knowledgeable about health and care issues.

Minimum GCSE passes in Maths and English.

Excellent communication, organisational and IT skills.

Ability to work under pressure, motivate and flexible over 7‑day shift pattern.

Desirable:

Practical knowledge of the discharge process.

Experience dealing with challenging situations, including anger, aggression and complaintsli>
