Responsibilities
Northwest Specialty Hospital, an extension of Northern Nevada Medical Center, brings expanded services to a new building located at 6225 Sharlands Ave. in Northwest Reno. The facility will offer acute rehabilitation and adult behavioral health services. Our acute rehabilitation services include occupational, speech, and physical therapy for adults recovering from stroke, injury, or another disabling condition. We offer advanced treatment for a wide range of conditions including stroke, brain injury, amputation, neurological disorders, major multiple traumas and spinal cord and back injuries. All patients will experience private rooms, have access to a newly renovated therapy gym, and personalized care from our qualified staff. The adult behavioral health services offered is an expansion to our current 50 and older program at NNMC. The new program will offer behavioral health services to all adults 18 years and older. The department is managed through a multidisciplinary team including physicians, nurses, CNAs, social workers, and therapists. Patients will experience an individualized care plan, inclusive of individual and family therapy, activities, medication management, and a 24‑hour secure, compassionate environment. This dynamic individual will plan and organize the rehab patient's admission process. He/she works collaboratively with the team to optimize patient outcomes and maximize reimbursement for the organization; assists with the accurate processing and collection of clinical data and subsequent submission to CMS via the UDS‑MR software; participates in developing and implementing marketing strategies that parallel the admissions process in collaboration with the Director of Rehabilitation Services. Job Duties/Responsibilities
Coordinates the referral development system designed to facilitate referrals to the unit. Conducts thorough pre‑admission assessments of rehab candidates from phone inquiries, faxed referrals and onsite assessments when appropriate. Makes acceptance recommendation to the medical director and/or director of rehabilitation services. Collaborates with in‑house staff to offer rehabilitation services to patients at NNMC who qualify. Coordinates with staff to transfer patients promptly to the rehabilitation unit to ensure optimal patient care.
