Run day-to-day operations for CalAIM ECM and supportive housing services in your county
Lead and support a team of Care Managers (training, coaching, performance, coverage)
Own documentation quality and deadlines; ensure program compliance (including HIPAA and funder requirements)
Manage program performance: track outcomes, analyze data, and improve workflows
Build strong relationships with community partners, funders, and property managers/owners
Manage program budget and reporting with accuracy and accountability
Provide outreach as needed (street outreach, engagement, assessments, navigation)
Carry a small caseload (~15 members) including service plans, file maintenance, and ongoing follow-up
Help members secure housing documentation, apply for vacancies, and connect to deposit assistance
Transport clients as needed and operate company vehicles per policy
What were looking for
2+ years successful care coordination/case management experience with housing-related services 35 years program management experience (including staff supervision and program improvement) Knowledge of supportive housing models, housing resources, and community systems Strong written and verbal communication; able to produce clear documentation and reports Comfortable working with diverse populations and complex needs Experience using EMR/EHR systems Valid drivers license + reliable insured transportation (agency insurer authorization required) Education: High school diploma/GED required; Bachelors in social services or related field preferred
