Provide individualized physical therapy treatment plans for patients. Conduct thorough assessments to evaluate patient needs and progress. Set and achieve personal performance goals aligned with company targets. Document patient progress and treatment outcomes accurately and timely. Stay updated on the latest physical therapy techniques and practices. Engage in continuous learning opportunities to enhance professional skills. Collaborate with healthcare professionals to optimize patient outcomes. Educate patients and families on treatment plans and home exercises. Utilize data and metrics to track patient progress and treatment effectiveness. Participate in team meetings to discuss patient care strategies and improvements. Maintain a clean and safe working environment for patients and staff. Experience/Qualifications

Valid state licensure as a Physical Therapist (PT). Commitment to continuous learning and professional development. Proven experience in developing individualized treatment plans. Strong assessment skills to evaluate patient needs effectively. Ability to work in a results-driven environment with high visibility. Ability to document patient progress accurately and timely. Experience collaborating with healthcare professionals for optimal outcomes. Strong communication skills for educating patients and families.
