Community Health Outreach Registered Nurse - Part Time - Variable
Job Summary
The Community Outreach Nurse is responsible for providing community education and health screenings in the community. Works under the general direction of the Population Health Director, or designee.
Core Job Responsibilities
Educate communities about healthy lifestyles and disease prevention.
Identify potential collaborative community outreach projects with relevant community agencies and act as liaison between the agency and the hospital.
Perform free health screenings for common diseases, including diabetes, hypertension, stroke, women's health and fall prevention.
Take vital signs and perform other screening tests.
Conduct health assessments, provide brief interventions, triage patients, provide support and follow up in navigating health and social service systems.
Make appropriate referrals and assist clients to make necessary arrangements.
Develop and complete program evaluations, documentation, reports and follow up as required.
Counsel people who are at increased risk of illness or injury about how to take precautions to prevent illness, including performing screenings that lead to early detection of disease.
Describe how insurance works and help uninsured or underinsured people gain access to healthcare services.
Lead community outreach by distributing public health information and provide training at schools, churches, workplaces, and other community settings.
Obtain all clinical trainings as required.
Design and set up off site pop up locations including tables, chairs, and tents.
Attend weekly taskforce meetings to develop community outreach activities.
Perform other duties as required.
Education/Experience Requirements REQUIRED:
Graduate of an accredited school of Nursing
Excellent verbal and written communication skills
Ability to communicate with diverse populations
PREFERRED:
Bachelor of Science in Nursing with related clinical and teaching experience necessary.
Licensure/Certification Requirements REQUIRED:
Current RN licensure in NY
PREFERRED:
Stroke Certified Registered Nurse (SCRN)
