Duration: 3 Months+ (possibility of extension)
Pay Rate:
$55/hr. on W2
Shift:
Monday–Friday | 8:00 AM – 4:30 PM
Job Summary: The Registered Dietitian is responsible for developing, implementing, and overseeing the nutrition services program in close collaboration with the Interdisciplinary Team (IDT). This role provides comprehensive nutrition care to participants, ensures regulatory compliance for menu planning and meal service delivery, and serves as a liaison between the organization and food and supplement service providers. The Registered Dietitian promotes organizational values, teamwork, and mission while supporting high-quality, participant-centered care.
Key Duties & Responsibilities:
Conduct comprehensive nutrition assessments to identify participant needs, preferences, and goals
Actively participate in IDT meetings and contribute to individualized care plan development
Deliver and document nutrition interventions per physician orders and participant care plans, including special diets, meal intake monitoring, and nutrition counseling
Oversee menu planning and meal delivery programs to ensure safety, regulatory compliance, and accuracy of special diets
Coordinate and monitor nutritional supplements as ordered
Provide training to day center staff on meal delivery and nutrition-related processes
Collaborate with physicians and IDT members to support smooth care transitions across settings (hospital, SNF, home, etc.)
Support intake and enrollment processes to ensure timely participant onboarding
Monitor participant status and promptly communicate changes to the IDT
Maintain compliance with scope of practice and legal authorization requirements
Complete all documentation accurately and within required timeframes
Maintain participant and organizational confidentiality in accordance with policies
Participate in quality improvement initiatives, staff meetings, and required trainings
Communicate regularly with leadership regarding workload, priorities, and program needs
Complete all mandated trainings and occupational health screenings on time
Maintain flexibility in work assignments as business needs evolve
Perform other duties as assigned
Required Skills & Experience:
Minimum
4 years of experience
managing nutrition services in a similar setting
At least
1 year of experience working with frail or elderly populations
Demonstrated ability to mentor and develop team members
Strong organizational, communication, and documentation skills
Ability to work independently with minimal supervision
Proven ability to prioritize effectively in a fast-paced environment
Experience working with individuals from diverse cultural and socioeconomic backgrounds
Strong commitment to serving vulnerable senior populations
