Overview
The Care Coordinator (CC) has a key role in managing patient outcomes and decreasing utilization in the primary care setting. This role focuses on supporting medically complex and high-risk patients by managing transitions of care from acute and post‑acute settings and providing proactive outreach, care coordination, and health coaching.
The Care Coordinator works collaboratively with providers, care teams, and external partners across our Clinically Integrated Network (CIN) to ensure seamless care delivery, patient engagement, and alignment with value‑based care objectives. This is a full‑time, onsite position based in our Verona, NJ office.
Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES
(included but not limited to):
Manage patients experiencing transitions of care (inpatient, emergency department, or skilled/short‑term acute rehabilitation) within 48 hours of discharge, ensuring timely implementation of the discharge plan of care.
Conduct clinical, functional, and psychosocial assessments; perform medication reconciliation; and coordinate referrals, including visiting nurse services (VNA), behavioral health, and other community resources.
Collaborate with providers, care team members, and fellow Care Coordinators to identify, stratify, and manage high‑risk patients, developing individualized care plans with patient‑centered self‑management goals.
Document all care coordination activities, interventions, and outcomes accurately and timely within the electronic health record (EHR).
Utilize data from multiple technology platforms, including EHRs, ACO partners, payer portals, hospital systems, and web‑based applications—to support population health initiatives and meet value‑based care requirements.
Coordinate and monitor ancillary services, including home care, hospice, durable medical equipment (DME), infusion services, and other post‑acute supports, ensuring services are delivered as ordered.
Implement population health and preventive care strategies using technology‑based tracking tools and evidence‑based guidelines.
Engage patients and caregivers through education, health coaching, and goal‑setting to promote self‑management, adherence to care plans, and improved health outcomes.
Maintain professional competencies by participating in continuing education and ensuring all licensure and certifications remain active and in good standing.
Qualifications REQUIRED SKILLS & QUALIFICATIONS
Current, active, and unrestricted New Jersey Licensed Practical Nurse (LPN) license
Advanced Education preferred but not required.
Certified Case Manager (CCM) certification preferred.
Minimum of 3 years of experience in care coordination, case management, direct patient care, home care, or health plan/insurance settings preferred.
Strong clinical judgment with the ability to analyze patient data and apply case management principles effectively.
Excellent interpersonal and communication skills, with the ability to influence, motivate, and engage patients and interdisciplinary care teams.
Proven ability to prioritize and manage competing responsibilities in a fast‑paced, high‑volume environment.
Demonstrated initiative, problem‑solving skills, and sound decision‑making, particularly in complex or sensitive situations.
Proficiency in EHR systems and comfort navigating multiple technology platforms.
WORK ENVIRONMENT/PHYSICAL DEMANDS
Office setting, with extended periods of sitting and computer work.
Travel required as needed to office locations and off‑site locations for meetings, trainings or care coordination activities.

The anticipated base annual salary for this role ranges from $65,000 to $85,000.
This position may be eligible for an annual bonus opportunity based on individual performance and company financial performance.
