is seeking a dedicated
Speech Language Pathologist (SLP)
to join our skilled nursing and rehabilitation team. In this role, you’ll help residents strengthen communication, improve swallowing, and enhance cognitive abilities while working in a supportive, resident-centered environment in Fort Wayne, Indiana. Responsibilities – What You’ll Do
Evaluate, diagnose, and treat speech, language, voice, swallowing, and cognitive‑communication disorders Develop and implement personalized treatment plans tailored to resident needs Provide one‑on‑one and group therapy sessions to support functional communication and safe swallowing Document assessments, daily notes, and progress accurately in the EHR system Collaborate with physicians, nurses, occupational therapists, physical therapists, and families to ensure coordinated care Educate residents, caregivers, and staff on communication strategies and swallowing techniques Ensure therapy practices comply with Indiana regulations and Kingston’s standards of care Qualifications – What You Bring
Master’s degree in Speech‑Language Pathology from an accredited program Certificate of Clinical Competence (CCC‑SLP) preferred; CFY candidates considered Experience in skilled nursing, rehab, or long‑term care preferred (new grads welcome) Strong clinical, documentation, and communication skills Compassionate, resident‑first approach with a focus on positive outcomes Why You’ll Love Working at Kingston Care Center of Fort Wayne
