Dartmouth Health is recruiting a Social Worker – MSW, Care Management. This role focuses on providing assessment, counseling, resource coordination, advocacy, and documentation for patients and families in need.
Base pay range $56,430.40/yr – $87,464.00/yr
Pay and incentives Sign‑on bonus eligible: $7,500
Responsibilities
Complete psychosocial assessments of patients and families.
Provide counseling to individuals and families to solve social and emotional problems.
Coordinate community resources to address identified needs.
Assist families adapting to complex care needs and agency coordination.
Provide consultation, assessment, referral and coordination for suspected child or adult abuse or domestic violence.
Document objective, concise clinical data in the medical record.
Perform other duties as assigned.
Qualifications
Master's Degree in Social Work (MSW) required.
Experience in an acute, home care or ambulatory setting preferred.
Strong organizational, verbal and written communication skills.
Sound decision‑making and judgment.
Time‑management and negotiation skills.
Conflict‑resolution ability.
Basic computer proficiency.
