General Description The Resident Service Coordinator (RSC) serves as a service facilitator to residents, supporting independence, self-reliance, and the ability to age in place. The RSC provides informal or limited case management and counseling; information and referral services; plans educational programming; and coordinates volunteer opportunities.

The RSC establishes and maintains partnerships with community service agencies, negotiates affordable services when needed, and educates residents on available resources. The position also monitors services delivered by third-party providers to ensure quality, appropriateness, and timeliness.

Working collaboratively with housing managers and property staff, the RSC empowers residents while avoiding the creation of unhealthy dependency and does not assume management or property administration responsibilities.

Responsibilities

Assist and advise residents and families regarding services necessary to maintain a self-reliant lifestyle

Promote wellness activities for all residents

Educate residents, families, and staff on available community resources

Assist residents in building informal support networks with peers and family members

Serve as a liaison between residents, community agencies, and service providers

Collaborate as a team member with housing management staff in serving residents

Encourage residents to proactively address social, psychological, and physical needs

Facilitate access to services using the least restrictive interventions possible

Assist residents or coordinate training related to understanding lease and tenancy obligations

Maintain clear boundaries by not performing duties assigned to property management

Specific Responsibilities

Monitor delivery of services to ensure they are appropriate, timely, and satisfactory

Perform service management functions for residents requiring assistance

Provide informal or limited case management, including assessment of social, psychological, and physical needs and development of service plans when services are not otherwise available

Educate residents on service availability, application procedures, and resident rights (individually and in group settings)

Report all suspected abuse to the appropriate agencies in accordance with legal requirements

Establish and coordinate volunteer support programs with community organizations

Advocate and, when appropriate, negotiate on behalf of residents for timely, affordable, and effective services, with an emphasis on on-site service delivery

Assist residents with applications for benefits and entitlement programs and support access to transportation

Meet with service providers as needed

Develop and maintain a comprehensive directory of community services

Assist management and third-party providers in identifying residents who may need services

Note: The Resident Service Coordinator does not provide continuous direct support services, does not perform property management administrative duties, and is not an activities director. Educational programming that enhances independence and self-sufficiency is appropriate.

Administrative Responsibilities

Document contacts with residents, families, and service providers

Maintain confidential resident files, including intake information, annual reviews, service termination documentation, abuse reports, and meeting notes

Ensure all resident records are securely stored to protect confidentiality

Complete accurate and timely reports for management, funders, and government agencies as required

Seek additional service resources through private, local, state, and federal sources

Develop service management plans as appropriate

Qualifications

Bachelor’s degree preferred with concentration in Social Work, Gerontology, Psychology, Public Health, or a related field

Demonstrated knowledge of community services and resources relevant to the resident population

Experience in service coordination, facilitation, advocacy, and problem-solving

Training or experience related to aging, disability services, mental health, and substance or medication misuse

Knowledge of federal and state entitlement programs and eligibility requirements

Awareness of legal and liability issues related to service coordination

Strong communication, writing, organizational, and advocacy skills

Appropriate professional license where applicable
