Description The ICM Oncology Coordinator supports Oncology RN Case Managers by providing non-clinical care coordination for Medicare Advantage patients with advanced cancer and high-acuity needs. Under the direction of the RN Assistant Director, this role focuses on timely referral and authorization processing, care coordination across the continuum, and identification of social barriers and care gaps that may impact treatment adherence. The coordinator works closely with oncology nurses to support safe transitions of care, prevent avoidable hospital utilization, and ensure patients remain on track throughout their cancer journey.

Key Responsibilities

Coordinate and process urgent, routine, pre-service, and retro authorization requests in compliance with UCLA Medical Group, CMS, DOFR, and health plan requirements

Provide non-clinical care coordination for oncology patients with high-acuity needs across inpatient, SNF, outpatient, and post-acute settings

Monitor hospital and SNF admissions and discharges; conduct post-discharge outreach and health screenings to identify risks, barriers, and care gaps

Collaborate with Oncology RN Case Managers to elevate high-risk issues and support development of non-clinical care plans

Coordinate transitions of care, including hospital-to-SNF and SNF-to-home services such as transportation, DME, home health, and ancillary services

Track referrals, authorizations, and patient activity; maintain accurate documentation in the electronic medical record

Collaborate with concurrent review nurses, ICM nurses, providers, and external programs to support continuity of care

Support quality improvement initiatives, reporting, appeals, and grievances as assigned

Salary Range: $35.47 - $48.82/Hourly

Qualifications Required

High school diploma, GED or equivalent

Experience processing Inpatient and ambulatory Managed Care referrals

Ability to multi-task, work with frequent interruptions, and meet deadlines. Must be detailed, oriented, attentive, organized, and able to follow directions

Proficient computer skills including working knowledge of Microsoft Excel and Word

Ability to communicate thoughts and information clearly and succinctly in writing as well as verbally

Ability to operate a wide variety of office equipment, including computers, printers, copy machines, facsimile receiver/transmitter, scanners and mailing equipment

Preferred

Minimum of 2 years’ experience working in a Managed Care Case Management and care coordination environment
