Veterans Transitional Housing Program (VTHP) Case Manager Program Type : VA Grant & Per Diem (GPD) – Transitional Housing

Reports To : Director of Supportive Services

The VTHP Case Manager provides intensive, client-centered supportive services to Veterans experiencing homelessness in compliance with VA Grant & Per Diem (GPD) and HUD Continuum of Care (CoC) requirements. This role supports housing stability, income maximization, service engagement, and successful exits to permanent housing.

Case Management & Supportive Services

Complete intake, eligibility verification, and program enrollment documentation within three (3) business days.

Maintain accurate and timely HMIS data entry in accordance with HUD HMIS Data Standards and VA reporting requirements.

Conduct biweekly case management meetings and develop, implement, and update Individualized Service Plans (ISPs).

Coordinate referrals to VA services, mainstream benefits, and community-based resources.

Provide budgeting and financial literacy support.

Provide crisis response, de-escalation, and on-call emergency coverage.

Documentation, Compliance & Reporting

Maintain complete and audit-ready HMIS case records.

Collaborate with the VA GPD Liaison to complete quarterly interim reviews.

Report critical incidents involving emergency services or law enforcement within 24 hours.

Complete VA discharge and exit documentation within two (2) business days.

Collect and submit weekly census data to the GPD Liaison.

Document participant noncompliance through written warnings and discharge notices utilizing the program's designated forms and documentation procedures.

Participant Engagement & Program Activities

Plan and facilitate quarterly participant engagement activities.

Support recovery-oriented goals and program engagement.

Property & Facility Coordination

Coordinate maintenance needs and unit turnover.

Mediate participant conflicts and support Resident Managers.

Professional Development & Collaboration

Participate in supervision, training, and compliance activities.

Collaborate with staff, VA partners, and community providers.

Basic Requirements Candidates must demonstrate proficiency in Microsoft Office for applications such as Word, Excel, and PowerPoint. Additionally, they should efficiently identify and utilize resources to assist participants with various tasks, including completing online applications, drafting resumes, accessing accounts, enrolling in benefits, gaining access to examinations, and other community resources that offer training or goal-oriented support.

General Skills

Strong reading and writing skills are essential.

Provide references and pass background checks aligned with position requirements.

Must be capable of lifting to 25 pounds if necessary.

Proficiency with Word and Excel spreadsheets is required for this position to meet the program's documentation and record-keeping requirements.

Experience Preferred experience in providing supportive services within the community. Preferably with a non-profit agency.

Education A bachelor's degree in social work, human services, or a related field is required, or an equivalent combination of education and demonstrated experience providing direct supportive services to individuals experiencing homelessness or housing instability.
