NON-RESIDENTIAL CARE MANAGER – PROJECT: ELEVATE Schedule:
Monday – Friday, 8‑hour shifts (no remote days) Compensation:
$28.85 per hour (non‑exempt) Job Description
Care Manager utilizes participant‑centered, trauma‑informed, harm reduction and strengths‑based approaches to empower individuals to seek liberation and reduce recidivism. The role works with individuals in a Community Corrections facility setting, collaborating with partners and other community‑based service providers to ensure participants have low‑barrier access to services. Care Manager is knowledgeable in the many avenues of healing from trauma and substance use disorders while implementing culturally responsive, gender‑responsive, and trauma‑informed care. Project: Elevate – Core Responsibilities
Providing intensive, holistic care management and transition planning services for residents serving a sentence in a Community Correction setting. Understanding and implementing the Project: Elevate Scope of Work, completing all required assessments, documentation, care plans and reports in accordance with Community Corrections Standards. Managing the transition of participants who have completed "in‑house" level requirements to "non‑residential" status, ensuring continuity of care and stability during the reintegration process. Conducting ongoing case management for non‑residential participants, including verifying employment, monitoring budget/financial obligations, and ensuring adherence to curfew and community location guidelines. Performing required community checks (home visits and employment visits) to verify the safety and suitability of non‑residential placements. Evaluating participant readiness for decreased supervision and identifying risk factors that may require a regression to residential status or increased interventions. Communicating and collaborating with operations staff, partners, and other community‑based service providers to ensure participants have low‑barrier access to services. Maintaining accurate and timely data in compliance with Community Corrections Standards. Identifying and connecting participants with appropriate referrals to community mental health and supportive services providers and permanent housing resources. Conducting behavioral interventions in accordance with evidence‑based practices. Other Responsibilities
Attending all required meetings. Obtaining all necessary supervision and training. Maintaining self‑care. Other duties as assigned. Qualifications
Must have a valid driver’s license or state identification. Lived experience navigating the Criminal/Legal Justice System is preferred. Restorative Justice knowledge and skills are preferred. Working understanding of harm reduction theory, trauma‑informed services, and restorative justice. At least 2 years of experience working with a diverse population with behavioral health needs (5 years preferred). At least 3 years of Case Management experience.
