Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Their mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
PACE Program
The PACE program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Nutrition Coordinator
Responsible for supporting participants’ overall health through comprehensive nutrition care. The role includes assessing nutritional status, developing and monitoring individualized nutrition care plans, and providing nutrition education and counseling to participants and caregivers. The nutrition coordinator works closely with the interdisciplinary care team to integrate dietary needs into each participant’s plan of care, oversees or coordinates meal services and menu planning to meet medical and regulatory requirements, and monitors ongoing nutritional outcomes such as weight changes, food intake, and diet tolerance to ensure meals and interventions support participants’ health, independence, and quality of life.
Qualifications Required
Prior work in nutrition coordination, food service management, dietary services, or related nutrition roles.
Understanding of nutrition principles, special diets, dietary restrictions, and food safety standards relevant to older adult populations.
Food safety certification (or willingness to obtain)
Ability to work collaboratively with an interdisciplinary healthcare team (e.g., nurses, dietitians, care coordinators).
Preferred
Experience working in healthcare settings (especially with older adults, chronic health conditions, or long-term care).
Experience assisting with nutrition education programs or group activities
Familiarity with culturally appropriate meal planning or accommodating diverse dietary preferences (often valued in senior nutrition roles).
