Comprehensive patient assessment and continuous telemetry monitoring in PCU. Administration and management of medications, IV therapies, wound care, and skilled nursing interventions tailored to patients transitioning from acute to step-down care. Collaborate with physicians, case managers, and interdisciplinary teams to develop and execute individualized care plans, anticipate complications, and deliver evidence-based interventions. Guide families through discharge planning; coordinate with pharmacy and therapy services; participate in quality improvement initiatives.
