Job Description
The Case Manager RN will be responsible for coordinating care for patients within the hospital's acute care settings. This role requires collaboration with interdisciplinary teams to create and implement effective discharge plans and ensure patient safety. Responsibilities
Assist patients and families in developing and fulfilling safe discharge plans. Coordinate with various service providers for post-acute placements. Fulfill documentation requirements in the Epic system. Support patients with durable medical equipment and home health needs. Work with community partners to arrange care plans and secure necessary authorizations. Required Experience / Certifications / Licensure
1 year of nursing experience; previous charge experience is preferred. Experience with Epic charting system is required. Current BLS certification is a must. Why ARMStaffing?
