Job Description PsychPros, Inc. has a full-time, temporary Medical Social Worker assignment available with hospital system in Los Angeles, CA, with travel required to skilled nursing facilities in Los Angeles County.
Assist patients and their families with care planning and discharge planning.
Complete psychosocial assessments.
Partner with patients to identify needs and develop and implement individualized treatment plans based on mutually agreed-upon goals.
Discuss options for care proactively, including medical center resources and external community/government resources to assist patients and families in developing short- and long-term care plans as appropriate.
Qualifications
A master's degree in social work.
Active BBS registration as an ASW.
One (1) year of recent internship experience in a healthcare setting.
Hospital acute care experience preferred.
