The Case Manager supports the department by providing case management to the different units within the facility. Manages the client’s cases with a solid background of clinical knowledge and expertise and may provide some therapeutic services to clients. Responsible for the overall discharge and aftercare planning needs and placement of the client. Maintains routine contact with referral sources and providers while maintaining resource file. Participates in quality care monitoring for the department. This position is onsite at the facility and is not a remote position.
Pay Range: $22.00-24.00 per hourly, based on experience.**Roles and Responsibilities:**ESSENTIAL FUNCTIONS:· Participates in the development of the service plan.· Implements appropriate therapeutic activities and/or programs. Implements services to clients in accordance with their individualized service plan (ISP).· Collaborates with the client and client’s family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option.· Maintains the client’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning) and client’s progress toward their recovery. Coordinates releases and faxing records to providers.· Pursues best practice outcomes for client with mental illness including accessing psychiatric services, therapy services, continuing education, employment, independent housing or other placement options and community tenure.· Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with client, their family, behavioral and general medical and dental health care providers, community resources and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented.· Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources; Ensures all covered services identified on the service plan are evaluated and updated monthly.· Coordinates referral or completes the screening and assessment on all clients on caseload for financial entitlements (AHCCCS, SSI/SSD etc.); completes AHCCCS applications on all clients on caseload meeting criteria.· Assures that discharge planning begins at admission. Obtains collateral input from providers and supports.· Ensures the development and implementation of transition, discharge, and aftercare plans prior to discontinuation of behavioral health services.· Ensures all contact with referral source and/or other treatment providers is done within designated time frame and documented accordingly.· Makes arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.· Employs appropriate crisis intervention techniques as needed.· Communicates appropriate client information to the treatment team; participates in team discussion and evaluation of client goals, objectives and motivations.· Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted.· Schedules after care appointments for clients with PCPs, psychiatrists, next level therapy programs, living arrangements, etc.ADDITIONAL RESPONSIBILITIES:· Complies with organizational policies, procedures, performance improvement initiatives and maintains regulatory and industry standards.· Maintains regular and reliable attendance.· Accepts responsibility for professional growth and development of self and attends all mandatory facility in-services and staff development activities as scheduled.· Maintains professionalism and confidentiality of facility employees and clients.· Performs other duties as assigned.CULTURAL EXPECTATIONS:· Honesty: Works with integrity and fairness.· Accountability: Promote trust, ensure responsible behavior, and contribute to team success.· Responsibility: Accept responsibility for work product and performance.· Adaptability: Receptive to giving and receiving feedback.· Collaboration: Work effectively with team members, leadership and/or external sources to achieve goals.· Competence: Strives to be the best and hold selves accountable.· Perseverance: Meets challenges with the resolve to succeed.· Passion: Cares about what we do, each other and who we serve.· Respect: Honors the human dignity of everyone we work with.· Teamwork: Succeeds, and have fun, by pulling together.EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:· Bachelor’s Degree in a behavioral health area, marketing, business administration or related field preferred.· One or more years previous related experience in Behavioral Health preferred.· Previous experience in case management and/or utilization review preferred.LICENSES/DESIGNATIONS/CERTIFICATIONS:· CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).· First aid may be required based on state or facility requirements.Veterans and military spouses are highly encouraged to apply.
