Social Worker – Southbridge Rehabilitation & Health Care Center (Southbridge, MA)
Pay:
$34.00/hr – $38.00/hr (dependent on license and experience).
Hours:
8:00 am – 4:00 pm, Monday–Friday.
Responsibilities
Plan, organize, develop, and direct social services to meet the medically‑related emotional and social needs of residents on an individual basis.
Coordinate and monitor resident care conferences.
Work with individual residents, responsible parties, and families toward adjustment to placement and to crisis situations.
Compliance with facility Privacy Practices, policies, and procedures while protecting residents’ individual health information.
Complete comprehensive documentation in a timely manner.
Advocate and coordinate personal needs for residents.
Utilize community resources appropriately.
Support staff in relation to resident‑related issues.
Perform other duties as deemed appropriate and assigned by the supervisor.
Qualifications & Experience
Must possess a social work license in the state of Massachusetts (LSW, LICSW, LSWA, and/or LCSW).
A minimum of one (1) year of social work experience in a hospital, skilled facility, or other health‑related field (preferred).
Must hold a bachelor’s degree in social work from an approved school of social work (minimum).
Specific Requirements
Fluency in reading, writing, speaking, and understanding the English language.
Ability to make independent decisions when circumstances warrant such action.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.
Patience, tact, cheerful disposition, enthusiasm, and willingness to handle residents of all maturity levels.
Openness to seeking new methods and principles and incorporating them into existing social service practices.
Ability to relay information concerning a resident’s condition/incident to the interdisciplinary team as appropriate.
