Overview
The OB RN Care Coordinator is a Registered Nurse who provides comprehensive, patient-centered coordination of obstetric care across the prenatal, intrapartum, and postpartum continuum. This role ensures that every birthing person and family receives timely, equitable, and evidence-based care through proactive assessment, communication, and collaboration with the interprofessional team.
The OB RN Care Coordinator promotes seamless transitions of care by facilitating early identification of risks, supporting individualized care plans, reinforcing education, and ensuring follow-up for medical, psychosocial, and behavioral health needs—including perinatal mental health concerns. Using clinical expertise, population-based thinking, and relationship-centered communication, the coordinator serves as a consistent point of contact for patients, families, and providers.
Requirements Minimum Education Graduation from an accredited school of nursing required.
Bachelors of Science of Nursing preferred
Minimum Work Experience Prior OBGYN/WCS nursing experience required
Required Licenses & Certifications Vermont State Registered Nurse License required.
Health Care Provider BLS certification required within 90 days of hire.
NRP and ACLS required within 6 months of hire.
