Job Overview
The Stars Coordinator plays a vital role in promoting and facilitating proactive healthcare management for our members. This is essential for improving member health outcomes and is a key factor in achieving a high Medicare Star Rating. This position is responsible for a comprehensive process that begins with outreach and education to the members, progresses to scheduling and coordination, and concludes with meticulous follow-up and record retrieval. The ideal candidate will possess exceptional communication skills, a deep understanding of the importance of preventive medicine, and a highly organized and empathetic approach to member engagement.
Essential Responsibilities
Proactively contact members, primarily via phone, to discuss and explain the benefits of specific preventative medical procedures (e.g., annual physicals, mammograms, colonoscopies, etc.).
Encourage and motivate members to schedule appointments by highlighting the long-term health benefits and addressing potential barriers.
Maintain a positive and empathetic demeanor, building trust and rapport with members.
Assist members in navigating the scheduling process, offering multiple options and accommodating their preferences for time, location, and provider.
Send appointment reminders and preparation instructions to members via their preferred communication method (e.g., email, text message, phone call).
Collaborate with clinical staff and external provider offices to ensure a seamless scheduling experience.
Follow up with members after their appointments to confirm attendance and collect feedback.
Contact provider offices directly to request and obtain relevant medical records, test results, and visit summaries.
