Salary:

$50,000-$52,000 annually (based on experience and credentials)

Hours:

8:30-4:30, Monday-Friday; flexibility as needed outside of routine schedule (37.5 hours per week)

Location:

Riverhead, NY and travel throughout Suffolk County

Position description The Health Home Plus Adult Care Manager will assist with navigating complex systems of care, build supportive relationships and develop individualized person-centered care plans to help individuals meet their overall health and wellness goals. The Health Home Plus Care Manager serves adults, 18 and over, with a Serious Mental Illness (SMI) who have experienced multiple hospitalizations, criminal justice involvement, homelessness, and/or have active Assisted Outpatient Treatment (AOT) orders. Work is done primarily in the community and the position entails travel throughout Suffolk County. The manager will have a mixed caseload comprised of both Health Home Plus and Health Home clients, delivering four services per month to high-acuity members (two in person and two telehealth).

Key responsibilities may include, but are not limited to:

Coordinating and monitoring behavioral, physical health, and social services based on consumer-driven needs assessment and service plan;

Building and monitoring linkages with natural supports and formal community resources including hospitals, outpatient clinics, rehabilitation, self-help, and social service providers.

Providing supportive, recovery and wellness-directed coaching and counseling in-person and via telephone communication.

Providing support to ACM team and clients when case coverage is needed.

Maintaining accurate records and following Department of Health, Office of Mental Health, Health Home, and agency policies and guidelines.

Participating in team meetings, supervision, Adult Care Management staff meetings, and agency meetings.

Qualifications

A Bachelor’s degree in a human service/health discipline and two years’ experience working directly with the SMI population. Candidates with a Master’s degree and one year’s experience are strongly encouraged to apply.

Two years’ experience in providing direct services to people with Serious Mental Illness (SMI), developmental disabilities or substance use.

Excellent interpersonal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric, substance use challenges, trauma as well as chronic physical health conditions;

Strong communication skills and collaboration with professionals involved in the person’s care;

Understanding of the key elements of recovery and wellness, including the role of social supports including family and self-help; PROS, etc.;

Sound organizational skills including computer skills (agency uses electronic health records, multiple IT platforms).

Knowledge of the public health/mental health system, public entitlement and social service programs;

Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
