Hospice team , delivering personalized, one-on-one care.
What You’ll Do As a Hospice LPN, you’ll provide hands-on clinical care and support to patients in their homes, while working closely with a multidisciplinary team. You’ll help manage care plans, monitor, and ensure each patient receives the compassionate and skilled care they deserve.
Your Day-to-Day Might Include
Providing direct nursing care, including treatments, wound care, and medication administration
Monitoring patient conditions and reporting changes to the RN or care team
Supporting the development and adjustment of individualized care plans
Collaborating with nurses, therapists, and other healthcare professionals
Educating patients and caregivers to promote independence and self-care
Accurately documenting all care provided and patient updates
Participating in team meetings and interdisciplinary coordination

What You’ll Need
Valid Rhode Island LPN license (required)
Equivalent education required for RI LPN licensure.
At least 1 year of progressive clinical experience (home health or acute care preferred)
Current CPR certification
Valid driver’s license and auto insurance
Strong clinical, communication, and documentation skills
