Regional Homecare Nurse

Summary
As a Regional Homecare Nurse, youll deliver high-quality, patient-centered nursing care across vibrant communities in the Bronx, Westchester, Manhattan, Queens, Brooklyn, Long Island, and beyond! Youll conduct routine home visits, provide wound care, administer medications, and perform health assessmentsmpowering patients to live healthier, safer lives in their own homes.
Travel within your designated area, making meaningful connections
Work independently with the support of an innovative organization
Enjoy a consistent schedule: 8:30 AM - 5:30 PM, 40 hours/week

Responsibilities
Perform wound care, medication management, and health assessments during home visits
Educate patients and caregivers on disease management, safety, and medication adherence
Collaborate with care teams, partner organizations, and physicians
Document all care accurately in our state-of-the-art electronic health records
Travel safely between patient homes, ensuring top-tier care in every visit
Qualifications
Active NY RN license (BSN preferred)
3+ years of clinical nursing experience, including home care or similar settings
1+ year working with elderly or frail populations (training provided if needed)
Strong clinical assessment, critical thinking, and communication skills
Ability to travel within your assigned region
Flexibility for weekends, nights, and on-call shifts
Proficiency with computers and electronic health records
Valid NY driver license and reliable vehicle (preferred)
Other Requirements
Ability to lift up to 50 pounds and navigate stairs
Comfortable working in diverse environments
Medically cleared and up to date on immunizations
