The LPN Case Manager will be responsible for completing any clinical assessment for patients requiring post-acute services under State Medicaid programs including, but not limited to, Virginia, West Virginia, and Maryland. The LPN Case Manager will be responsible for completing pre-screens, admissions, and insurance authorizations for Valley Health’s skilled nursing facility and transitional care beds.Extra duties include, but are not limited:Assisting Case Management staff with completion of Level 2 (MIMR) assessment.Interviewing patients, family members, and/or caretakers as needed to complete assessments.Maintaining open communication with care management team regarding approval/denial of clinical assessments.Maintaining proficiency in electronic submission programs and awareness of current policies/trends related to clinical assessments for Medicaid services.Assists with the arrangement and follow-up with community resources. LPN Case Manager will assist in providing a seamless transition utilizing the entire continuum of care and helping to facilitate patients movement to the next level of care.**Education**Graduate of an accredited school of nursing required**Experience**Two years bedside experience required**Certification & Licensure**Licensed as a Licensed Practical Nurse in the State of Virginia requiredBLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved is required. \* New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.**Qualifications**Competent to successfully interact with patients according to their special needs which may apply based on the age of the patient served.### **Benefits**At Valley Health, we believe everyone is a caregiver, and our goal is to create an environment where our caregivers thrive physically, financially, and emotionally.
