We are currently seeking an experienced and detail-oriented Clinical Team Coordinator to join our Boca Raton office. Position Overview The Clinical Team Coordinator plays a vital role in ensuring the seamless coordination of patient care services. This role requires a proactive individual with experience in home health operations, scheduling, and Medicare compliance.
Key Responsibilities
Coordinate patient care in collaboration with referral sources, patients/families, the clinical team, and management.
Track admissions/discharges from higher levels of care and ensure timely updates in documentation.
Obtain and enter physician orders for continuation or resumption of services.
Monitor visit frequency requests and ensure patient-specific schedules align with agency guidelines.
Request and track necessary documentation from hospitals and physician offices for QA/QI compliance.
Schedule labs and diagnostic tests with outsourced partners; report results and update patient records.
Accept and enter telephone, fax, and email orders into the EMR.
Initiate and manage wound care tracking plans in collaboration with clinicians.
Support clinician communication and respond to patient updates as needed.
Assist with onboarding of new hires and preceptor coordination.
Provide backup support to the administrator and perform additional tasks as assigned.
Qualifications
Active Florida LPN or RN license required.
Minimum of 2 years of skilled home care experience (field or office).
Strong knowledge of Medicare home health guidelines and industry best practices.
Proficient in Microsoft Outlook, Teams, and Excel.
Experience with Forcura is a plus.
Excellent communication, organizational, and multitasking skills.
