Position Summary
Licensed Practical Nurse (LPN) responsible for providing direct care, medication administration, and support to residents at Viva Senior Living. Must hold an active LPN license issued by the State School of Nursing.
Essential Functions
Comply with federal, state, and local health and regulatory standards and guidelines.
Maintain compassion, dignity, and adherence to the Resident Bill of Rights.
Adhere to Viva Senior Living’s attendance policies and employment terms outlined in the Employee Handbook.
Read, write, speak, and understand English.
Maintain high standards in caring for residents and their families.
Follow all facility policies and procedures.
Responsibilities and Accountabilities
Provide care, treatment, and medication administration to assigned residents, monitoring condition and reporting per plans of care.
Assist in oversight of ancillary care staff when absent of Wellness Director or Assistant.
Provide personal care and assist with Activities of Daily Living (ADL).
Administer medication in accordance with the Five Rights and maintain records.
Reorder and receive medications.
Supervise care staff and maintain shift-to-shift reports.
Provide emergency assistance and obtain/transcribe physician orders.
Maintain case management documentation.
Requirements
Valid LPN license issued by the State School of Nursing.
High school diploma or GED.
Ability to work well with peers in a congenial environment.
Must be able to read, write, speak and understand English.
Must adhere to all facility policies and procedures.
Annual verification of a negative TB skin test.
Proof of full COVID‑19 vaccination (exemptions for medical and religious accommodations).
Physical and Sensory Requirements
Move intermittently throughout the workday.
Coping with mental and emotional stress of the position.
Ability to see and hear or use prosthetics to meet required senses.
Meet general health requirements, including a medical and physical examination.
Interact with ill, disabled, elderly, emotionally upset, and occasionally agitated residents.
Push, pull, move, or lift a minimum of 25 pounds to a height of 5 feet and carry same weight 50 feet.
Assist in evacuation during emergency situations if necessary.
