Home Health Speech Therapist
Prairie River Home Care, Inc. is looking for a Home Health Speech Therapist to provide speech and language therapy to clients in an outpatient setting. The Speech Therapist will plan and give care under physician direction, work with families, nurses, and other team members.
Responsibilities
Administers speech and language evaluations and basic audiological assessment, participates in Plan of Care development.
Establishes treatment goals, maintains current care plans, reassesses clients and makes recommendations.
Provides speech therapy treatments, procedures, evaluations, and diagnostic tests under physician direction.
Maintains records of visits and treatment plans.
Guides therapy with all care team members; instructs client and family on home programs.
Participates in case conferences and in‑service programs.
Prepares clinical and progress notes; reports to physician and Area Clinical Manager on client response or condition changes.
Provides educational programs on speech and language to clients and families.
Maintains confidentiality and safe client environment; reports suspected abuse or neglect.
Performs other related duties as assigned.
Coverage Area
Reports to St. Cloud office; provides services in Waite Park, Clearwater, St. Joseph, St. Cloud and surrounding cities.
Schedule
Part‑time position with flexible schedule during office hours Monday–Friday.
Requires traveling to clients’ homes for 1:1 care sessions lasting 1–3 hours per client.
Physical/Environmental Demands Light to medium physical demands, requires driving or public transport; occasionally lifts up to 20–50 lbs.
Benefits
Pay: $40–$55 per hour, $65–$75 per visit.

Qualifications
Accredited Speech‑Language Pathology program graduate with master’s or higher.
Certified by ASHA and licensed in Minnesota with no restrictions.
Minimum 2 years healthcare experience, preferably home care.
Strong interpersonal, teaching, organizational, and independent work skills.
Access to reliable vehicle or public transportation; valid driver’s license, proof of insurance.
Negative Mantoux or chest X‑ray documentation.
Current CPR certification highly recommended.
U.S. citizenship or valid Alien Work Permit.
Clear background check and no disqualifying criminal history; must pass ongoing background studies including MD Health and Feds.
