This position provides direct care and medication assistance to residents in a manner that meets or exceeds community expectations.
Pay Range Base pay: $18.00/hr - $20.00/hr.
Responsibilities
Personal Care Assistant to Residents.
Laundry/Housekeeper duties.
Dishwasher/Kitchen helper.
Bus person.
Essential Functions: Medication Administration
Maintain security and accountability for medications during shift.
Inventory controlled substances and provide accurate reconciliation.
Count controlled substances with another associate each time responsibility changes.
Review medication administration record for errors at completion of each medication pass.
Ensure safe medication administration following state licensure rules.
Verify residents who are approved to administer their own medication do so and complete the medication administration record.
Assist in reviewing medication books for errors.
Complete medication error forms and obtain associate signatures before forwarding to supervisor, including physician notification.
Responsible for medication reorders, including PRN medication.
Weekly review of medication sheets and verification of residents’ capability to administer their medications.
Fax written or faxed orders from doctors to pharmacies.
Check residents’ files daily for appropriate and necessary signatures.
Complete incident reports with physician and notify family members as directed.
Provide assistance with activities of daily living (grooming, oral hygiene, bathing, hair cleaning, transfers, meals).
Assist with residents’ personal environment (clothing, linen, personal belongings).
Additional Responsibilities
Respond appropriately to changes in the work setting.
Be familiar with Crescendo Senior Living Policies & Procedures Manual and Associate Handbook.
Perform other related assignments as required.
Attend required in‑service training.

Report suspected or observed abuse, neglect or misappropriation of property or funds to appropriate licensing agencies.
Perform duties under the direction of a licensed nurse within scope of certified practice and verified skills competency.
Remain awake pursuant to state regulations.
Qualifications
Minimum 18 years old.
Pass required health screening, TB clearance, and criminal record clearance.
High school diploma or equivalent preferred; experience in similar facility valued.
Valid First Aid certification; may need current CPR, Certified Nursing Assistant certification or state certifications; must pass requirement examinations and maintain continuing education.
Fluency in English (speaking, reading, understanding).
Effort & Physical Requirements
Maintain mental alertness, attention, and concentration for necessary periods.
Apply common sense understanding and carry out instructions (written, oral or diagrammatic).
Adapt to situations with precise attainment of set limits, tolerances, or standards.
Regularly sit and talk or listen.
Ability to stand, stoop, bend frequently; lift up to 75 lbs and push up to 50 lbs.
Handle, feel, and operate tools or controls; reach with hands and arms.
Vision requirements: close vision, depth perception, ability to adjust focus.
Adequate physical stamina to move freely and assist residents, including emergencies.
Working Conditions
