Job Summary This position works closely and individually with families to support their success in our center-based and Learning at Home programs. This floating role requires travel to all schools when needed. Responsibilities include partnering with families of enrolled children to facilitate family success in goal setting, child health records and medical home documentation, referrals for emergency support, monitoring compliance requirements, and supporting home language as necessary. The role helps families develop Child Health Plans & Individual Health Development Plans, navigate community health services, support family engagement, and manage a caseload of families, ensuring regular touch points and submission of required documentation.

Responsibilities / Essential Functions

Family Enrollment Meetings: Provide personalized support to families of newly enrolled children by collecting required documents, discussing program expectations, and ensuring effective communication and strong relationships through ongoing follow-up.

Screening and documentation review: Ensure compliance with regulatory requirements; obtain consents and releases, review and analyze medical documentation, assist with examinations, and perform necessary follow-up screenings (WCC, Lead, Sensory, Growth, Dental).

Referrals: Refer clients to medical/dental providers; foster relationships with families, children, school staff, and community partners; coordinate financial assistance plans if families cannot pay for services.

Record-Keeping: Submit health screening results, maintain confidential records in ChildPlus, state immunization, and lead databases; compile weekly and monthly reports on screenings, exams, and follow-up progress.

Special health needs: Implement and oversee care plans for children with special health care needs, monitor health records, and contact parents regarding due dates.

FSP and Referrals: Assess family strengths, complete Family Success Plan, collaborate with families and teachers to set goals, provide emergency support referrals, and follow up after absences.

Parent Programming and Involvement: Encourage parent involvement in programs, organize and promote family connection events, facilitate group connections, and provide resources on community events.

Home language support: Support parents in Spanish/Zomi as needed (interpretation services, ELL screenings, diet meetings, Multidisciplinary Team meetings).

Perform other duties as assigned or requested.

Education / Experience

Associate’s degree in social work or a closely related field (human services, family services, counseling, etc.), or equivalent years of work experience. Bachelor’s degree preferred.

CFLE (certified family life education credential) within 18 months of hire unless holding a relevant degree.

2 years of related work experience.

Bilingual (English/Spanish or English/Zomi) preferred.

Certification / License

Must obtain a food handlers permit and CPR/First Aid Certification within 30 days of employment.

Must obtain state certification for vision screening and medication administration within one year of employment.

Knowledge / Skills / Abilities

Proven interpersonal relationship skills to maintain effective working relationships with staff, vendors, clients and contractors.

Able to work collaboratively and respectfully with families.

Able to follow oral and written instructions and present information efficiently.

Able to audit records and ensure compliance with guidelines.

Take initiative, multitask, and work well under pressure.

Maintain strict confidentiality of sensitive data.

Intermediate MS Office skills and data system proficiency (ChildPlus).

Advanced organizational skills, efficiency, and attention to detail; commitment to timely completion.

Understanding of Head Start performance standards and OECP program requirements.

Essential Physical & Mental Requirements Employee must stand, stoop, walk, lift, and carry objects. Must lift up to 50 pounds. Required to travel across Tulsa County, handle all weather conditions, frequent driving.
