Day Program Case Coordinator
– $26-$27/hr

What You’ll Do
Develop and maintain individualized day service plans, updating throughout the year.
Coordinate semi‑annual reviews of service plans with families, service coordinators, and clients.
Communicate with clients and providers to ensure plans meet client needs, providing guidance or direct intervention as needed.
Oversee HICSIS reporting, ensuring all timelines and documents are entered.
Audit and maintain client case records.
Provide direct care support to program staff as needed (10–20 hrs/week).
Maintain day‑service vehicle fleet records across all programs.
Meet with staff routinely, verify data tracking in each room, and confirm completion.
Keep families or guardians informed of client progress or concerns.
Participate in licensing or certification of program sites.
Ensure all records are kept up to date, in paper or electronic format.
Assess clients’ strengths and needs, develop ISP (Individual service plan), and report progress toward objectives.
Develop methodologies for teaching specific objectives and long‑term goals.
Other duties as needed.
Qualifications
Bachelor’s degree preferred; minimum of 3 years’ experience in a day‑service setting with case‑management responsibilities.
Proficiency with Microsoft Excel.
Strong writing, typing, and interpersonal communication skills.
Excellent organizational skills, able to meet reporting timelines and deadlines.
Valid driver’s license.
