Care Coordinator, Acute Social Worker II

Position Summary
Collaborate with the clinical team to identify patients who would benefit from care coordination services, including risk assessment, utilization management, and preventative care.
Develop and monitor discharge plans, and work with Patient and Family Counselors, Social Workers, and UR nurses to engage families, advocate for patient needs, and facilitate seamless transition across inpatient, emergency, and outpatient settings.
Provide education to patients and families on chronic illness self‑management, health care system navigation, and available resources such as prescriptions, durable medical equipment (DME), and community services.
Utilize IT resources (e.g., Phytel, Crimson) to maintain patient registries, track outcomes, and support evidence‑based clinical decision tools.
Participate in population health and clinical outcome measurement initiatives, ensuring patient safety and compliance with Orlando Health policies and applicable laws (ADA, FMLA, etc.).
Offer clinical interventions under supervision of a licensed Mental Health Therapist, including psychosocial support, group facilitation, and mental health education.
Maintain accurate documentation and adhere to quality standards across all care settings.
Perform other related duties as assigned.
Qualifications Education/Training
Master’s degree from an accredited School of Social Work, Mental Health, Psychology, or Marriage and Family Therapy (required).
Licensure/Certification
BLS certification.
Experience
Two (2) years of direct clinical experience focused on the population served, or completion of a Master’s level internship within the population (substitutable).
