Position Summary Reporting to the Sr. Director, the Community Engagement Specialist works to promote Careforth’s Structured Family Caregiving program, building relationships with local Care Coordinators, Area Agencies on Aging and other organizations/partners, and to generate interest in and referrals for Structured Family Caregiving. Additionally, the Community Engagement Specialist works locally, in collaboration with care coaches and other professionals, to support family caregivers who are caring for their loved ones at home. The Community Engagement Specialist visits families at home, conducts family-centered assessments, and identifies the support a caregiver needs to help provide care at home. This individual partners with a centralized coaching team to ensure ongoing engagement, support, intervention as needed, and collaboration with Care Coordinators.

What You Will Do

Build collaborative partnerships with local community organizations to promote the benefits of Structured Family Caregiving and to help identify and engage caregivers and families that need support

Represent Careforth at provider, advocacy and partnership meetings, as necessary

Conduct in-person visits with caregivers and consumers (care recipients) in their homes; capture assessment and other notes systematically

Work amongst a team to support caregiver needs across territories in the state

Assist caregivers in compiling credentialing documents necessary to enable program participation; support caregiver to set-up required mobile communication app, if needed

Collaborate with internal care coaches regarding assessment and ongoing coaching activities, making recommendations for coaching interventions

Perform other duties as assigned

What You Will Bring

Bachelor’s degree in social work, or an equivalent combination of education and experience in related fields

Minimum two (2) years’ experience in case management and program operations in the areas of healthcare and social services, preferably working with people with disabilities

Strong and culturally competent communication, relationship building, and assessment and problem-solving skills

Proficiency with case management systems and mobile apps

Working knowledge of home and community-based services and delivery systems

Ability to build strong collaborative relationships and community partnerships across the State

Ability to travel (usually by automobile) within assigned geographic area up to 80% of the time throughout Central North Carolina; valid driver’s license and automobile insurance is required

The pay range for this position is $50,342 - $62,218. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
