is currently seeking a
Licensed Medical Social Worker
to join our team to serve our patients currently enrolled in our
hospice services
in the
Albuquerque, NM region and surrounding areas.
The successful candidate will provide coordinated care to patients of all age groups, performing psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient, client, and family. This position offers an opportunity to make a significant impact on the lives of patients and their families in a supportive and dynamic environment.
Requirements
Conduct ongoing clinical assessments of hospice patients to identify psychosocial, financial, environmental, and community resource needs.
Develop and evaluate the plan of care in collaboration with the patient, client, representative (if any), and caregiver(s).
Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
Utilize effective interpersonal relations and communication skills to facilitate positive outcomes for patients and their families.
Collaborate with the interdisciplinary team to promote coordination of patient care and ensure comprehensive support.
Participate in the Agency's Quality Assessment and Performance Improvement (QAPI) program and in-service training.
Stay updated on public and private eligibility standards and requirements, maintaining professional growth and competency.
Promote the Agency's philosophy and administrative policies to ensure the delivery of quality care.
Requirements
Master's or Doctoral Degree from an accredited school of Social Work.
Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
Minimum of one year experience as a Master of Social Work (MSW) in a healthcare setting.
Working knowledge of community resources and good interpersonal skills.
Proof of current CPR certification.
Reliable transportation with a valid driver's license and auto liability insurance.
