Licensed Practical Nurse (LPN)
and play a critical role in delivering compassionate and effective patient care within our dynamic healthcare program.
Key Responsibilities
Patient Assessment and Care Planning: Assist in assessing patients’ health status and contribute to individualized care plans.
Direct Patient Care: Provide safe, effective nursing care for multiple patients within the facility’s program.
Patient Education: Implement education initiatives based on established teaching plans.
Nursing Scope of Practice: Perform delegated tasks consistent with education and within scope, use a problem‑solving approach for clinical decisions.
Care Coordination and Monitoring: Oversee onsite and offsite scheduling with the care team, ensure follow‑up care including telemedicine appointments.
Quality Improvement and Collaboration: Participate in quality improvement initiatives and patient care committee meetings; coordinate with regional medical and nursing directors for seamless care.
Documentation: Monitor CARES and ensure proper coordination, perform additional nursing duties as assigned to support outcomes.
Key Qualifications
Educational Background: Graduate from an accredited Licensed Practical Nursing program.
Licensure and Certification: Current LPN license in the state of employment and active CPR certification.
Knowledge: Familiarity with state laws governing LPN practice; successful completion of initial and annual clinical competencies.
Other Requirements: Ability to pass background check, security clearance, and drug screening.
